高雄長庚外科部長郭耀仁~亞洲首例 異體換臂 新手能握杯


這是一大好消息。曾來匹茲堡UPMC受訓過的臺灣高雄長庚醫院的郭耀仁醫師,他在2004~2005年曾來參加台灣同鄉會聚餐活動。我也幾次報導過他回來領獎的消息。

台中一名失去左前臂的45歲男子,今年9月初在高雄長庚完成手臂移植,手術歷經11個半小時,成功讓一名腦死患者的左手臂「移」愛到他的斷臂上,此舉創下亞洲首例手臂異體移植,代表台灣器官移植邁入新階段!據了解,當男子看到左手「重現」,感動地與父母抱頭哭泣,經過復健,他的新左手已能握杯與騎單車。

據蘋果日報報導,高雄長庚去年初獲衛生福利部核可,計畫進行5例手臂異體移植人體試驗,今年9月有一名因病身亡的36歲男子,由妻子同意捐出包含左手在內的全身可用器官,促成手臂移植手術,整場手術長達11個半小時,高雄長庚出動整形外科、骨科、麻醉等多科協力,醫療團隊取下捐贈者左前臂後,以顯微手術縫合22條肌腱、6條靜脈、3條神經與2條動脈到受贈男子斷臂處,為讓移植後組織能存活,單是0.1公分血管就需縫8至10針。當手術完成的那一刻,參與醫師形容:「站到體力潰散,內心卻是狂喜。」

據報導,該名男子住院一個半月後重回職場,每周有三天南下高雄長庚做復健。他的新左手從手指完全不能動,到現在已能握起杯子,甚至騎單車外出,讓30年沒用過左手的他感歎:「恍如隔世。」

高雄長庚外科部長郭耀仁是移植手術主刀者和靈魂人物,他師承中研院士、長庚大學醫學系教授魏福全與美國約翰霍普金斯大學首席整外名醫李為平,李為平前年底曾為一名在伊拉克戰爭中失去四肢的美國大兵完成雙臂移植,名揚國際,出身高雄的李為平得知郭耀仁完成台灣首例手臂異體移植,表示非常高興。

報導指,帶領長庚手臂移植團隊的魏福全指,依國際異體移植學會登錄資料,全球有70多人、107隻手臂的異體移植個案,台灣新紀錄已向該學會網站申請登錄。一位不具名整形外科醫師表示,中國、馬來西亞也曾做異體手臂移植,但未在醫學會發表,長庚若完成登錄,台灣將成文獻中首例完成換手移植的亞洲國家。

衛福部醫事司長王宗曦指,異體手臂移植屬人體試驗,高雄長庚需依試驗內容完成五例後再送審,才可望成為常規手術。目前高雄長庚執行異體手臂移植實驗,手術費用加術後復健與五年抗排斥藥費共400多萬元台幣,由院方全額支付,未來四例,長庚也會全部支付。至於納入常規手術後,部分費用可由健保給付,被移植者花費目前無法估算。

據報導,至於手臂異體移植的風險,高雄市立聯合醫院院長、手外科名醫林高田指,帶皮膚的手臂排斥比其他器官高,被移植者須終生吃高劑量抗排斥藥,可能造成免疫力降低,甚至提高罹癌機會,「是否有必要為不危及生命的事移植」,是斷臂者需思索的問題。

全美首名在戰爭中失去四肢但幸運存活下來的士兵,去年12月中旬接受由台灣移民醫生李為平(W. P. Andrew Lee)率領的醫療團隊,進行罕見、創新但成功的雙臂移植手術,術後情況良好,雙臂目前已可稍微移動。

戰中失去4肢仍存活 
接受雙臂移植的是26歲的士兵布蘭登.馬洛可(Brendan Marrocco),他於2009年在伊拉克被1枚路邊炸彈炸傷失去四肢;去年12月18日在馬里蘭州巴爾的摩約翰霍普金斯醫院接受13小時的雙臂移植手術,馬洛可在術後1個月的1月18日曾上推特透露,新雙臂「已經可以動一下」。

此一移植手術涉及骨頭、血管、肌肉、肌腱、神經、皮膚與兩條手臂,意即馬洛可不僅接受已故捐贈者的兩條手臂,還有對方的骨髓細胞,藉此讓馬洛可在使用最低量藥物的情況下,身體可以不排斥這兩條新手臂。約翰霍普金斯醫院指出,此一療法看似奏效,不但成功避免新雙臂遭排斥,也減少使用的抗排斥藥物。

這種創新的免疫壓迫治療法的發明人與雙臂移植手術的主刀醫師,正是15歲時從台灣移民美國的李為平;儘管過去也曾動類似手術,但李為平稱馬洛可的手術是至今最複雜的,得花1年以上才能知道馬洛可能多全面使用他的新手臂。李為平說,神經再生最快每月長1吋(2.54公分),完全復原少說要1、2年。

知名首席整形外科醫師李為平過去任職匹茲堡大學醫院時就做過3件類似手術,迄今已為5人移植手臂,術後復原情況都很良好,而且其中有4人只需要注射1劑抗排斥藥物,不像大多數接受器官移植的病患得接受混合藥物治療。儘量減少使用抗排斥藥物的重要性,在於抗排斥藥物有許多副作用,以及長期使用有增加罹癌風險之虞。

馬洛可的父親艾利克斯說,兒子是首位在伊拉克與阿富汗戰爭中失去四肢且存活下來的士兵,對於能有一雙新手臂非常高興;他們全家都要感謝捐贈者家屬的無私決定。

馬洛可的手術是全美第7宗雙手或雙臂移植手術。全球上一次成功進行雙臂移植是2008年在德國進行的一場手術。此外,馬洛可的手術獲得美國軍方的贊助,估計有約300名美軍在戰爭中失去雙臂或雙手。

高雄長庚醫院外科部主任郭耀仁,是美國斷肢移植權威李為平第一個台灣學生,兩人亦師亦友,郭耀仁在美學變臉、換手異體移植後,親筆書法送給李為平,李一直掛在辦公室,顯示兩人友誼深厚。

郭耀仁說,李為平小時候住在高雄岡山,李父是空軍飛官退役,後來全家移民美國,但很懷念在台灣的童年生活,3年前曾受邀到高雄長庚演講,期間他帶著李為平夫婦到高雄一遊,欣賞美麗風光。


李為平醫師 複合組織移植權威

郭耀仁說,李為平頭腦冷靜,行事穩重,在四肢異體移植手術領域是全球頂尖,美國軍方因許多士兵手腳殘缺,因此提供龐大經費給李為平做移植手術研究。

我首件異體移植 郭耀仁將操刀
郭耀仁於2004年在長庚大學魏福全教授推薦下前往美國匹茲堡大學,跟著李為平學習變臉、換手異體移植技術,日前也通過衛生署核准進行國內第一起異體移植手部的人體試驗,他計畫在5年內尋找5名手部外傷缺損或手部先天畸形的病患,進行換手人體試驗。

中研院院士魏福全指出,李為平原本在哈佛任職,被挖角到匹茲堡醫學院,近年才又回到他的母校約翰霍普金斯任職,也曾任美國手外科醫學會理事長。魏福全說,李為平在複合性組織移植領域還有一個非常重要的突破,即為了減少免疫抑制藥物的使用劑量及藥物種類,在進行肢體移植時合併進行骨髓移殖,讓身體對於移植物具有耐受性,因而得以使用單一免疫抑制劑。

李為平操刀 沉穩指揮數十醫護

李為平(圖)主刀完成馬洛科的雙手移植手術。翻攝網路為馬洛科完成雙手移植手術的主刀醫師李為平(Wei-Ping Andrew Lee),15歲從台灣移居美國,取得約翰霍普金斯大學醫學博士學位,現為手部外科、整形手術權威。手部移植手術工程浩大,需動員數十名醫師和護士,但李為平指揮若定,在手術室展現大將風範。

高雄出生哈佛畢業
李為平的台灣好友,中研院院士及林口長庚顯微重建外科醫師魏福全說,兩人自20多年前認識,李為平對於當時在整形外科已有聲譽的魏福全相當景仰,兩人一見如故,亦師亦友。 

高雄長庚醫院整形外科部長郭耀仁曾赴美向李為平學習,兩人至今還常交流研究心得,他透露李為平在高雄岡山出生,父親是退役空軍,李為平曾就讀台北市復興小學,15歲赴美求學,妻子也是台灣人。李為平自哈佛大學畢業後,就讀約翰霍普金斯大學醫學院。之後他專攻整形重建外科,擔任哈佛大學醫院主治醫師時開始專攻手部移植,後任職匹茲堡大學醫院整形外科主任時研究異體移植。 

已為5病患接新手
移植病患通常須終生服用抗排斥藥物,有時因排斥作用導致移植器官無法作用。李為平為減輕病患痛苦,研究出「免疫調控」(immune modulation);將捐贈者骨髓細胞植入受贈者骨髓,讓身體習慣新來器官,降低排斥,業界讚譽有加。李已先後為5個病患接上新手,並給予捐贈者的骨髓,5人都復元得很好。李

為平的研究團隊已獲美國防部軍隊再生醫學研究所贊助經費,擬利用他的「免疫調控」,來進行「變臉」-臉部移植手術。

李為平喜歡70年代迪斯可音樂。他家地下室有個閃閃發光的迪斯可球,3個子女有2個念的是老爸的母校哈佛。

中研院士魏福全︰複合組織移植 最怕身體排斥 


名列四百年以來「全球整形外科廿位權威」之一的魏福全醫師,他曾說過:「為了○.○1%的革命發現,99%的徒勞無功不算什麼!」(請參閱《今週刊》第812期2012-07-11 專文報導)

國人對器官移植算是耳熟能詳,手、腳、臉的移植在台灣卻是聞所未聞,難道是國內的醫學中心技術做不到?還是這類移植真的格外困難?

該手術比內臟移植更複雜
中研院院士、也是長庚醫院複合性異體組織移植中心的催生者魏福全醫師表示,手、腳、臉、腹壁等移植手術,正式名稱為「複合性異體組織移植」,從1998年迄今,全球手移植手術應該已經有80幾例,腳也有20幾例。

魏福全指出,「複合組織移植」在技術上跟斷指再接差不多,其中包括接血管、接神經等顯微重建技術,國內醫學中心其實沒有太大問題,長庚醫院進行相關準備也已有10年時間。先前陸續送了3名醫師到美國李為平醫師所在的中心進修,迄今沒有著手進行臨床移植,主要是考量到如何面對患者移植後,必須長期服用抗排斥藥物的問題。

他說:「這就好像是一個母親,面對可能生下一個先天免疫缺乏的孩子時,必須考慮到很多問題。這必須是對患者術後一輩子的承諾。」

魏福全指出,一般民眾熟知的肝、腎等器官移植,是為了搶救性命,相較之下,複合性組織移植主要是為了改變功能或維持尊嚴,但不論是異體器官移植還是複合性組織移植,都要面對術後的免疫抑制藥物使用,但皮膚的排斥高於腎臟,也高於肝臟。

過去已知當患者長期使用抗排斥藥物壓抑自身免疫時,可能提高糖尿病、腎衰竭,甚至腫瘤發生的機率。而當面臨的並非生命威脅時,是否要讓患者面對這些問題,就變成一個很重要的考量。

林口長庚醫院整形外科系主任林志鴻指出,肢體移植除需要龐大手術團隊外,患者術後的復健、術後終身用藥等,費用都很可觀,光進行1例移植就可能需上千萬元費用。

魏福全:為了○.○一%的革命發現,九九%的徒勞無功不算什麼!

七月五日,是台灣醫療學術史上的重要時刻,這天長庚醫院外科醫師魏福全,當選第二十九屆中研院生命科學組院士,是國內首位獲此殊榮的外科醫師,到底魏福全是如何辦到的?

「滴、滴、滴……,」踏入新北市林口長庚醫院的開刀房,耳邊傳來生命監測器規律聲響,被麻醉昏迷的病患躺在手術台上,手術醫護團隊就定位,來長庚受訓的外國醫師們往門口翹首盼望,他們在等一個人,就是世界顯微重建整形外科權威,長庚醫院前院長魏福全。

沒多久魏福全出現了,和往常一樣,身著手術衣的他,從容不迫走入開刀房,開始向醫師講解手術細節。不過此刻他多了一個新身分,中央研究院院士。

他 不是手術技術員,是醫師科學家
魏福全接受《今周刊》專訪時高興地說,「以前學界認為外科醫師就只是個做手術的技術員;其實我有很多原創性的學理研究,不但可以轉換到臨床應用,還輸出國外。我是個整形外科醫師,也是個『科學家』。」他加重語氣強調「科學家」,因為對魏福全來說,科學家這三個字是對外科醫師不會做創新研究的重大翻案,是他伏首案間三十年的最大肯定。

「他對全球整形外科界的貢獻實在很大,美國整形外科醫學會頒獎給魏福全,認為他是世界整形外科四百年歷史以來,最重要的二十位醫師之一。」國家衛生研究院分子與基因醫學研究組特聘研究員兼主任、中研院院士王陸海說。

魏福全能獲選為中研院院士,與他多項「世界第一」的創新手術與學理研究有關。他是全球做最多腳趾移植自手指手術,腓骨(小腿骨)骨皮瓣移植自口腔癌病患缺損部位,以及穿通枝皮瓣移植自缺損部位的醫師。另外,他帶領的團隊已經完成兩萬個病例,成功率高達九七.五%,就連哈佛、約翰霍普金斯等美國名校,都有醫師飛來長庚學習,哈佛醫學院教授Goldwyn盛讚,「如果沒有來過長庚整形外科,就不算完成整形外科訓練。」

他的學理研究和臨床手術一樣揚名世界。魏福全在最權威的月刊《整形外科顯微重建手術》(Plastic and reconstructive Surgery),已經刊登一百篇,論文總數已超過六百篇,被引用次數近八千次,「據我所知,台灣外科界只有魏院長有這樣的成就。」林口長庚整形外科系主任林志鴻說。


他 助少年移植斷指,重拾音樂家夢
十年前,一名英國少年湯瑪斯想要成為一位音樂家,但在一場意外中,他的中指被截斷,斷指葬送他學樂器的可能,湯瑪斯終日鬱鬱寡歡,直到他遇上到英國教學的魏福全。魏福全把湯瑪斯的腳趾接在斷指上,湯瑪斯終於可以開心學樂器。

英國的湯瑪斯,只是魏福全團隊上萬個病患中的一人,「接受移植手術後,完全沒有手的人可以開火車,斷指的人成為外科醫師。」魏福全說。

這樣的手術有多困難?人類的手有錯綜複雜的掌內肌肉、交纏的血管網絡和神經,手指、腳趾血管只有一公釐,而手術用縫針只有五十個微米(○.○五公釐),縫線已經細到肉眼難看見,全須借助放大倍率達十六倍的顯微鏡來操作。

「顯微鏡下平整的皮膚變成小山丘,手若是微微抖動,對病人來說就是大地震,在這種情況下,一條血管要縫上八針,還要保證百分之百的血管神經通暢率,每個步驟都是關鍵,稍有不慎,不但斷指不能復元,連切下來的腳趾都沒用了。」魏福全這樣形容他的工作。

除了腳趾移植手指術,魏福全也是腓骨(小腿骨一種)骨皮瓣移植下巴骨頭、手臂骨頭和大腿骨的專家,讓患口腔癌的卡車司機可以挖掉長在下巴的癌細胞,臉也不會缺一塊;車禍撞碎大腿骨的少女逃過截肢命運。「魏院長不是做這些手術的第一人,但卻創新改良發揚光大。」林志鴻說。

他 沒有立志做大事,卻義無返顧從醫
不過,對現在世人眼中的驚人成就,魏福全年輕時想都沒想過。一九四五年出生於屏東,家中經營海產山產買賣的魏福全,畢業於高雄醫學院醫學系。「我不是從小立志做大事那種,我們的教育總說要立大志,其實我不大相信,你沒有看過什麼叫大,怎麼會知道什麼叫大事……。」

「我當醫師是因為我爸媽要我當,走外科是因為我喜歡外科醫師豪爽的性格,走路醫師服都不用扣;走整形外科,是因為我景仰長庚前院長羅慧夫(羅為美國人,一九五九年自美來台,是脣顎裂專家)。」魏福全一口氣就解釋完三十五歲以前的人生重要抉擇。

「在長庚的時候,羅院長叫我參加一場國際論文發表會,在那次國際研討會中,發現自己學術表現不輸其他國家。」魏福全開始把同業當作假想敵,在不斷超越對手中,得到成就感。被肯定越來越多次的他,對名聲漸漸上癮,並變成驅動他不斷向前的一股可怕意志力。 


他 用三十年、八千個病例,超越對手
從一隻小白老鼠的血管開始練習對接血管,三十年來,魏福全累積八千個病例,醫術也不斷精進;但是追逐成功的代價也很大。

「連開十二小時的刀,凌晨兩點下開刀房,隔天早上七點繼續上班。有一次什麼東西都沒吃就光喝啤酒,結果暈倒,現在已經有胃潰瘍的毛病。」「在十二小時的開刀後,不是馬上休息,而是建檔研究一兩個小時,反省剛剛的手術過程……。」魏福全平靜描述以前的工作情況,卻讓人看到他風光背後的辛苦。

除了意志力,真正讓他醫術與研究能揚名海外的,還有面對失敗案例的坦然,與忍受研究失敗的耐心。魏福全的手術就算有九七.五%成功率,還是有二.五%的失敗,「很多醫師不敢把病發案例拿出來討論,但我不會,我一定會拿出來討論,也許有點難堪,但這沒有關係。」

在魏福全近三十年的醫學研究生涯中,其實高達九九%的研究發現都沒有太大意義,只有一%有意義;一%有意義的研究中,可能只有○.○一%的研究有革命進展,但魏福全願意為這○.○一%的機會,忍受九九%的徒勞無功。

他 即使只剩自己,也要繼續走下去
碰到失敗與挫折時,魏福全也不敢說沒有過放棄的念頭,但他會不斷激勵自己,這是一場馬拉松賽跑,撐到最後的才是贏家,「我很佩服挑戰人類極限的人,每次看到他們創紀錄,心裡都很澎湃。」

不過,這麼一位外科技術登峰造極的人,最大的挫折,不是自己的失敗,而是病人的投訴或揚言提告。「我是沒有上過法院,但我學生被告是常有的事,二.五%的失敗就會被病人強烈責難;當然醫師也會失誤,可是有些是無法避免的。」

魏福全感嘆,因為外科容易發生醫療訴訟,長庚整形外科雖然全球知名,很多醫師受完訓練仍選擇離開自創醫美診所,願意留下的人越來越少。

但就算現在醫療環境惡化,他還是沒有停止繼續往前推進。講起下一個移植大趨勢「異體移植」,魏福全整個眼睛都亮了起來,「絕對不能以為自己很強,永遠要往前、往前,再往前。」
